Our client is actively searching for a Staff Nurse The responsibilities of this role are:
To perform home visits to palliative care patients who are being referred
To perform nursing assessment on the patients.
To initiate nursing care plan and implement new measures of management.
To adjust current medication to achieve maximum symptom control under doctor’s instruction
To work closely with the Medical Director/Outsourced doctors on the cases.
To educate caregivers/families on basic nursing procedures to ensure comfort and care
To refer patients to other professional bodies such as inpatient hospice when the need arises for symptom control or respite care.
To identify patients and families in need of financial assistance and refer to social worker/counsellor.
To identify patients and families in need of specialized counseling to the relevant professionals.
To prepare families for approaching death.
To provide family with the emotional and bereavement support when necessary
To participate in Fund raising activities when the need arises.
The ideal candidate will need to have the following qualities and experience:
Minimum of 3 years of related experience.
Diploma or degree in Nursing or equivalent, with a Nursing Certificate in Palliative Care.
Registered with the Singapore Nursing Board.
